Ship allowed to sail to a repair yard
single voyage or endorsed certificates issued
repair contract or supply of equipment confirmed
flag State (or RO)
confirm that voyage is permitted and any conditions (use form FUD 1)
Maritime Authority of Repair Port
advised and confirmation received that action will be taken (use form FUD 1)
Master
informed that port state control authority of repair port must confirm ship’s arrival (use form FUD 2)
master advised of banning possibility (use form FUD 2)
repair port and ETA noted on PSC form and FUD 2
“agreed repair yard” included in inspection report in information system if next port is in the Paris MOU
confirmation of departure sent to port state authority of repair port, flag state and recognized organization(s) (use form FUD 2)
Ship related message
inserted in the ship’s information system record to include name of repair port, ETA and summary of outstanding deficiencies
If Banning applied
MOU members, European Commission and Secretariat informed.
letter at Appendix 1, 2, 3 or 4 sent to Company and to
all copy addresses
